If after reading our information about outside online pharmacies, you would still like to have us approve a prescription for your pet through one of them, here are some important things for you to know.

We ask that you sign this form documenting that you have been informed about outside online pharmacies. You may return it by fax, by mail, or in person.

Due to liability concerns, we will no longer be communicating directly with the pharmacy companies via fax, phone, or mail.  After we have received your signature on the form listed above, we will be happy to write you a written prescription, similar to what you would receive  at your physician's office, for you to fill wherever you choose.  You may pick this up in person or we can mail it to you.

Just as if you were refilling your pet's medications through us, normal refill and prescription expiration regulations will still apply, as will the labwork/examination requirements that are applicable to ongoing refills on some medications.
